Experience and Qualifications:

Responsibilities:

Specific tasks include, but are not limited to: -Reviews inpatient diet prescriptions, reviews dietetic technicians’ nutrition screening worksheet, and consults with dietetic technicians on inpatient status. -Determines patients’ levels of nutritional risk, and determines priority of care for patients needing nutrition therapy intervention. -Reviews patients’ medical records and interviews patients, family, and medical staff to obtain information necessary for the development of nutrition care plans. -Develops and recommends nutrition therapy for inpatients and outpatients. Coordinates as necessary with medical, nursing, and Nutrition Care Division staffs. -Provides nutrition/medical nutrition therapy education to patients and families. -Completes diet calculations for in- and- outpatients, tailoring plan for each individual situation. -Recommends and understands specifics of Enteral products and Total Parental Nutrition (TPN) tailored to meet individual patient needs. -Monitors patient progress and adherence, provides follow-up education as needed. Advises physician when changes are warranted. -Utilizes automated systems, CHCS, AHLTA, and CIS, or a paper medical record to look up patient labs, meds, and records to document care given, the nutrition therapy plan and process appointments and data IAW established procedures. -Attends and participates in discharge planning and regular staff meetings -Coordinates and provides in-service training to physicians and other health care providers, and Department of Nutritional Medicine. -Teaches nutrition classes to outpatients, their families, and active duty military members.
